Like many health care private practice offices and health oriented small business-people, I do not bill insurance directly as reimbursement is negatively skewed against private practice.

However, some clients recieve reimbursement for medical nutrition therapy services

  • if your insurance plan provides the benefit for nutrition therapy, which may differ per diagnosis (CPT) code

  • and you have a written (paper or digital) referral for medical nutrition therapy signed by your health care provider (MD) detailing diagnosis along with either a statement of the desired objective of nutrition / dietary treatment, or a diet order.

In summary, Jill Madison/Middle Path Nutrition & Wellness is out of all insurance networks and is not responsible for verifying your insurance coverage or for billing your insurance carrier on your behalf.
