Like many health care private practice offices and health oriented small business-people, I do not bill insurance directly as reimbursement is negatively skewed against private practice.
However, some clients recieve reimbursement for medical nutrition therapy services
if your insurance plan provides the benefit for nutrition therapy, which may differ per diagnosis (CPT) code
and you have a written (paper or digital) referral for medical nutrition therapy signed by your health care provider (MD) detailing diagnosis along with either a statement of the desired objective of nutrition / dietary treatment, or a diet order.
In summary, Jill Madison/Middle Path Nutrition & Wellness is out of all insurance networks and is not responsible for verifying your insurance coverage or for billing your insurance carrier on your behalf.
